To simplify your first visit, please fill out our New Patient Form* online prior to your appointment or arrive 10 minutes early to fill it out in office.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
Initial Examination
Your dentist will review your medical and dental history and complete a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening and any x-rays that you may require.
Treatment Plan
Based on your examination findings, your dentist will develop a long-term dental treatment plan for you, and take the time to answer any questions you may have about your dental health and treatment options.

Insurance & Financing
At Yonge Street Dental, our administrative team will help you with payment and can assist with your insurance company.
- Insurance Assistance
Our administrative team can assist you in understanding your insurance plan better and can also send estimates to your insurance company prior to starting any major treatments.
Please bring your insurance plan information to your appointment.
- Payment Methods
Payment in full is required at the time of your appointment. Any co-pays or amounts not covered by insurance are your responsibility.
Our preferred payment methods are debit or most major credit cards.

- How can I book an appointment?
To book an appointment at Yonge Street Dental, please contact our office during our regular business hours.
- How do I cancel or reschedule an appointment?
If you need to cancel or reschedule an appointment, please call us. Please let us know about any change of plans as soon as you can, so we can offer the space in our schedule to another patient. We require notice of cancellation 2 business days in advance to avoid a fee.
- Do you provide services in any languages other than English?
Yes, our office is able to provide dental services in several languages including Italian and Portuguese.
If you have a language preference, please advise us when booking your appointment.
- Does your clinic have convenient parking?
Yes, there is convenient paid parking available for our patients in the underground visitors parking lot in the office building. We are also easily accessible by public transit.
